NATIONAL FORMS
- Official Bankruptcy Forms Official forms are approved by the Judicial Conference of the United States and are mandatory. They must be used to file a bankruptcy petition and take certain other actions in a bankruptcy case, such as filing a Proof of Claim or applying to pay a filing fee in installments.
- Procedural Forms Procedural forms are drafted and approved by the Administrative Office of the U. S. Courts. They are recommended for use, rather than mandatory.
LOCAL FORMS
Local forms are drafted and approved by the Board of Judges for use in the District of Delaware. If a form use has been designated as mandatory, failure to utilize the form may result in a delay in the processing of a case.
